d-Mannose, a Trojan horse against bacteria
d-Mannose is a simple sugar structurally related to glucose. Related compounds are in use in probiotic products in the form of mannan-oligosaccharides (MOS) with benefits similar in kind and in mechanisms of action to those of d-mannose in protecting the urinary tract. Mannans are constructed from mannose to form linear polymers or chains. MOS are long-chain non-digestible complex carbohydrates that look like the mannans expressed on cells that line the digestive tract. Often bacteria recognize these cell surface molecules and use them to attach to the intestinal lining, a first step in colonization. By ingesting MOS, this microbial attachment mechanism can be interfered with. Unfriendly bacteria bind to the ingested MOS hoping to get a foothold in the intestines, but instead they are tricked into a ride through and out of the digestive system.
d-Mannose performs the same trick on bacteria in the urinary tract. There are bacteria that have tiny projections containing lectins that act as cellular glue, binding these bacteria to the bladder wall which hinders dislodgement by urination. The attachment is to mannose receptors of the cells of the bladder wall. According to one authority, the “chemical structure of D-mannose causes it to adhere to (such) bacteria, perhaps even more tenaciously than (these bacteria) adhere to human cells.” This line of argument has been borne out by recent human trials utilizing oral supplements containing d-mannose. Work on the benefits for human urinary tract health of d-mannose involves a number of clinical studies and is on-going.
Cranberry for the urinary tract
Cranberries (Vaccinium macrocarpon) are rich sources of dietary flavonoids, including anthocyanins and proanthocyanidins, and contain both organic acids and unique phenolic compounds that help maintain the health of the urinary tract. Although no definite mechanism of action has been established, research suggests that cranberries prevent normally non-resident bacteria from adhering to the uroepithelial cells that line the bladder wall, preventing the disruption of the epithelial surface. Direct microbial balancing activity has been shown in vitro in the urine of subjects after ingestion of a commercial cranberry product. Furthermore, another study found cranberry juice to contain at least two inhibitors of lectin-mediated adherence of certain bacteria to eukaryotic cells.
Promotes a healthy urinary tract
A known review of 10 randomized controlled trials of cranberry products found an overall direct, microbial balancing effect of cranberry, in vitro. In a one year study of 150 women aged 21-72, subjects were randomized to one of three groups: placebo juice + placebo tablets, placebo juice + cranberry tablets, or cranberry juice + placebo tablets.
Both cranberry juice and cranberry tablets were found to statistically significantly decrease the number of subjects experiencing urinary symptoms due to bacterial adhesion, on a yearly basis, compared to placebo.
A second study of 10 Greek women aged 46–60 gave the subjects 400 mg cranberry capsules per day (two in the morning, two in the evening) with vitamin C capsules for 6 months; cranberry capsules were found to be highly effective at preventing urinary symptoms.
Finally, in a more recent randomized clinical trial from 2014, 928 participants given cranberry or placebo capsules twice daily for 12 months found participants with high risk at baseline had an improved incidence of clinically defined urinary symptoms with cranberry capsules than with placebo.
